air leaks tire valve stem

I have experienced air leaks at the base of the valve stem where it
contacts the rim on two different KLR's. In each situation, the
tires and valve inserts are fine. I am curious to know how the valve
stem fails (in case I am doing something to inadvertantly damage it)
to prevent this from happening again. Does anyone else have this
problem?

The obvious thing to check is that the hole in the wheel isn't rough
or sharp where the valve stem goes through. The tiniest metal burr
will work its way into a tube. Also, if the stem doesn't go straight
through the tube, whether from a bad install or movement after
installation, it could get stressed enough to leak eventually.
Beyond that, all I have is questions: Have you always run the same
brand of tube? Do you change them when you change tires? Do you live
in a high ozone area? Are there gremlins in your garage?
