What are the early symptoms, it's time to repack the muffler?

I'm at 43,000+ and things "SEEM OK".

I hear (read) some talk like its standard, like replacing a spark plug, or changing oil.

And of course the "clean out" plugs are pretty much worthless, since they don't come out. (lol)
